L\u2019objectif est \u00e9galement de favoriser leur int\u00e9gration sociale par le biais d\u2019activit\u00e9s communautaires.<\/p>\n<p>\u00c0 cette \u00e9poque, le r\u00f4le des intervenants consiste principalement \u00e0 observer, identifier et rapporter les difficult\u00e9s rencontr\u00e9es par les r\u00e9sidents \u00e0 l\u2019\u00e9quipe traitante. Ils ne participent donc pas activement \u00e0 l\u2019\u00e9laboration ou au suivi du plan d\u2019intervention. Ce changement implique de nouvelles responsabilit\u00e9s pour l\u2019ensemble des membres des Habitations Nouveau D\u00e9part. L\u2019implication des intervenants devient plus intensive, et les interventions, plus soutenues et fr\u00e9quentes.<\/p>\n<p>Le d\u00e9veloppement des aptitudes \u00e0 la vie domestique et \u00e0 la vie quotidienne devient alors une priorit\u00e9. Cela inclut, entre autres, la gestion budg\u00e9taire, l\u2019entretien m\u00e9nager, l\u2019hygi\u00e8ne personnelle, l\u2019alimentation, les habilet\u00e9s sociales, ainsi que toute autre comp\u00e9tence favorisant l\u2019am\u00e9lioration de la qualit\u00e9 de vie de la personne, le tout dans un logement propre et abordable, propice \u00e0 l\u2019int\u00e9gration communautaire.<\/p>\n<p>Il va sans dire que la concertation avec les partenaires du r\u00e9seau de la sant\u00e9 et des services sociaux demeure essentielle. Ce milieu de vie exige un niveau d\u2019encadrement particuli\u00e8rement \u00e9lev\u00e9, compte tenu de l\u2019intensit\u00e9 des apprentissages \u00e0 r\u00e9aliser dans un court laps de temps. L\u2019objectif est de favoriser l\u2019acquisition d\u2019habilet\u00e9s permettant une int\u00e9gration r\u00e9ussie dans la communaut\u00e9.<\/p>\n<p>Par ailleurs, la r\u00e9sidence Rose des Vents ouvre ses portes \u00e0 titre de milieu de vie semi-transitoire. Elle peut accueillir jusqu\u2019\u00e0 quatre personnes. Cette r\u00e9sidence repose sur les m\u00eames principes que la r\u00e9sidence St-Fran\u00e7ois. Elle accueille des personnes pour une p\u00e9riode transitoire, avec pour objectif le d\u00e9veloppement des habilet\u00e9s de base, notamment les aptitudes \u00e0 la vie domestique et \u00e0 la vie quotidienne. Cette centralisation vise \u00e0 regrouper les services administratifs \u00e0 une m\u00eame adresse tout en augmentant le nombre de places en logement offertes aux usagers. Une demande est formul\u00e9e aupr\u00e8s du CSSSG afin d\u2019obtenir le soutien d\u2019un organisateur communautaire. La capacit\u00e9 d\u2019h\u00e9bergement totale de cette r\u00e9sidence est de neuf personnes. La pr\u00e9sence des bureaux administratifs sur place permet d\u2019offrir un soutien plus intensif aux r\u00e9sidents.<\/p>\n<p>De plus, une salle communautaire est accessible pour la tenue d\u2019activit\u00e9s \u00e9ducatives et r\u00e9cr\u00e9atives.<\/div><\/div>[vc_empty_space height=\u00a0\u00bb20px\u00a0\u00bb]<div id=\"ultimate-heading-427169f9d61f06713\" class=\"uvc-heading ult-adjust-bottom-margin ultimate-heading-427169f9d61f06713 uvc-9872 \" data-hspacer=\"no_spacer\"  data-halign=\"left\" style=\"text-align:left\"><div class=\"uvc-heading-spacer no_spacer\" style=\"top\"><\/div><div class=\"uvc-main-heading ult-responsive\"  data-ultimate-target='.uvc-heading.ultimate-heading-427169f9d61f06713 h4'  data-responsive-json-new='{\"font-size\":\"\",\"line-height\":\"\"}' ><h4 style=\"font-weight:normal;color:#000000;\">2012<\/h4><\/div><div class=\"uvc-sub-heading ult-responsive\"  data-ultimate-target='.uvc-heading.ultimate-heading-427169f9d61f06713 .uvc-sub-heading '  data-responsive-json-new='{\"font-size\":\"\",\"line-height\":\"\"}'  style=\"font-weight:normal;color:#000000;\">\u00c0 la suite des changements apport\u00e9s et dans le souci de mieux d\u00e9finir les Habitations Nouveau D\u00e9part, l\u2019organisme a ressenti le besoin de faire le point sur sa mission, sa vision et ses services pour les ann\u00e9es \u00e0 venir. Il a donc \u00e9t\u00e9 convenu de faire appel \u00e0 une consultante afin de mettre en place une planification strat\u00e9gique. L\u2019un des \u00e9l\u00e9ments importants soulev\u00e9s lors de cet exercice a \u00e9t\u00e9 l\u2019importance de la croissance de l\u2019organisme.<\/p>\n<p>Dans le cadre des orientations du Plan d\u2019action r\u00e9gional en sant\u00e9 mentale (Sant\u00e9 et services sociaux Qu\u00e9bec, 2005-2010), un sous-comit\u00e9 h\u00e9bergement-logement, compos\u00e9 de repr\u00e9sentants du milieu communautaire et des Centres de sant\u00e9 et de services sociaux de l\u2019Outaouais, s\u2019est pench\u00e9 sur les besoins en mati\u00e8re de logement pour les personnes vivant avec un trouble grave de sant\u00e9 mentale. L\u2019organisme a b\u00e9n\u00e9fici\u00e9 de la premi\u00e8re subvention accord\u00e9e par l\u2019Agence de la sant\u00e9 et des services sociaux pour ce programme, ce qui a permis l\u2019embauche d\u2019une coordonnatrice.<\/p>\n<p>Gr\u00e2ce \u00e0 un partenariat avec la Fondation Immobili\u00e8re de l\u2019Outaouais, Les Habitations Nouveau D\u00e9part ont proc\u00e9d\u00e9 \u00e0 l\u2019ouverture d\u2019une nouvelle r\u00e9sidence en sant\u00e9 mentale dans le secteur de Gatineau. La r\u00e9sidence Berthe n\u2019a pas rouvert ses portes. \u00c0 la suite de cette situation, le conseil d\u2019administration, en concertation avec les membres de l\u2019\u00e9quipe, a pris la d\u00e9cision de faire l\u2019acquisition d\u2019une nouvelle r\u00e9sidence dont l\u2019organisme serait propri\u00e9taire.<\/p>\n<p>Par la suite, l\u2019organisme a apport\u00e9 des ajustements au fonctionnement et aux services offerts \u00e0 la r\u00e9sidence Maloney ainsi qu\u2019au milieu de vie situ\u00e9 au 137, appartement 5.<\/p>\n<p>De plus, \u00e0 la suite de plusieurs concertations avec diff\u00e9rents partenaires, un nouveau projet voit le jour chez Les Habitations Nouveau D\u00e9part.
